New Patient Forms

Our New Patient Packet is for people wishing to make Wellness Life Center their new PCP. If you are interested in having WLC be your new Primary Care Manager, please fill out the attached form and we will be in touch soon!

Weight loss Program Forms

If you are an established WLC patient, have been referred to WLC or are interested in starting our weight loss program but are an established patient at another practice please choose and fill out the appropriate form for your visit and we will reach out to you soon to get you set up for our weight loss program.
